Yabbut, what caused the fireworks, underdimensioned contacts, voltage
swing or something?

Too low contact area and insufficient contact pressure. Its also
possible that the contacts were not physically strong enough to resist
the motoring force exerted by the fault current.

We didn't design the relays. We just spec their ratings, both load and
through fault currents.
