Need Help Identifying PCB Mounted Transformer

Hi! I have a circuit board which has a small transformer mounted on it which I believe is bad. It has a sticker on it with '4080' on it. The Board runs on 24v DC and outputs 50v - 100v DC @100ma. This is a power source for an old gamewell fire alarm system. The gamewell circuit which hooked to the board shorted and got really hot, now I get 0v from the board.

Are you sure about those input /output levels..?
If so, it could be an early attempt at a dc/dc converter but doesn't appear to be enough components in the vicinity.
Also, in these, the tranny rarely, if ever, fails.

Show a top view of the complete board.
 
I have a manual for this thing and it does mention a DC to DC converter. Attached is the schematic.

Many of those types of transformer have a thermal fuse embedded in the windings, with some luck you can detect the two ends of the fuse and short them out to test, transformers seldom fail open, usaully shorted.
