I need to hire someone that can build a pulsed electro magetic field
therapy device.I became used to the PAPIMI at 54,000 volts for
1,000,000 th of a second.

Any sort of magnetic therapy is quackery, and 54KVolts could mean anything,
you can generate that sort of voltage with a simple automotive ignition
coil, but the total power will be low.

A sign transformer will wake you up good - for a second or two.

I saw an antique electro therapy machine once on the Antiques
Roadshow. It used a noble gas filled glass bowl as the probe. When
applied to the body a plasma discharge was seen inside it, similar to
that seen in large novelty plasma discharge globes. The current
discharge must have been insignificant but it probably was impressive
enough to make the patients believe they were being cured of whatever
ailed them.

You can pick those up on eBay. Usually under $100. :)

They are just line operated Tesla coils with various gas-filled glass
tubes or globes of various sizes and shapes to impress the ignorant.

It's a combination of a Tesla coil and a Geissler tube. Usually called a
Violet Ray or Violet Wand. BTW, the violet probes are partially evacuated
air filled.
