Yeah, four Form A contacts, two normally open, two normally closed are an unusual configuration. I've seen this in large contactors though. They could be wired as two Form C sets of contacts, but that may not be how these relays are used.
@dorke's suggestion to get a quote is probably going to yield a truly exorbitant price. The OP had better be 100% certain it needs replacement. If there is room, two or more relays with their coils in parallel could be used to emulate this relay's contact configuration. For example, four Form C contacts in two relays are common and can be wired to obtain four Form A contacts, two normally open and two normally closed. That leaves a lot of connections unused of course, but OTOH does increase the amount of space available for connecting wires.
