Is this due unmotivated flawed engineering?

More likely due to trying to squeeze the last penny out of the design. Such
efforts tend to make designs very sensitive to everything in the
manufacturing process going just right.

Maybe better start buying the ac adaptor on your own.. esp those nice 12V
versions.. Can't be worse :)

Where I work we have literally hundreds of Dell laptops that use
the affected adapters. None have blown up in the spectacular way that
seems to motivate the recall (?? I've never actually seen the supposed
failure mode mentioned ??), but many have failed due to the
wire or insulation failure at the plug's strain relief due to physical abuse
or just plain use.

Delta is a well-established Taiwanese company.

If there's a genuine design fault, the approvals process *should* have picked it up.

I'm wondering what 'overheat' means in the context of the supplied link.

A friend had a Mac/ Delta supply that was being recalled. Hers
smoldered during a law school exam to the point the proctors almost
called the fire dept.
