Debugging a sugar cube relay

I have a sugar cube relay which I am using with my controller. It turns On/Off when high or low signals are sent from the microcontroller. It can take upto 240V AC at 10 amps. The way the relay circuit board is that I connect the positive and negative of DC or AC Line and neutral at the input terminals, I get the positive and negative DC at the output terminal blocks or line and neutral at the output terminal blocks When I connected 120V AC Line and neutral wires at the input terminals of the relay, at the output I am only getting 12V AC. What could possibly the reason for this ? Is the relay broken ? It was working fine a few days ago.

Can you please draw us a diagram? This help to eliminates mistakes on our part, and considering the voltage you are working with, I would certainly call that a requirement.

I have attached a picture of the relay board here. The second relay on the right works fine. But the first one doesnt. I gave 120V Line and neutral at the input, and am getting only 12V AC at the output.

Sounds broken to me... Can you provide an image of the bottom of the board?
 
What I would do is first remove the AC side completely and check continuity from both in and out for 0 ohms when the relay is energized.
